Please click the link below to access your follow-up survey.  If you attended an Adult treatment program at Rimrock Foundation (e.g., adult chemical dependency, Michel’s House, Drug Court, Jail-Based) please use the “Adult Follow-up” survey link.  If you attended an Adolescent treatment program, please use the “Adolescent Follow-up” survey link.

Once in the survey, you will be prompted for your user ID and password.  Your user ID and password were provided to you in the letter or email you received from Rimrock Foundation.
